Using Workbooks to Support Advance Care Planning

Advanced care planning (ACP) is a process of thinking about, talking about, and sometimes documenting preferences and wishes for future care. ACP provides an avenue to include persons with dementia in care planning when capacities for expressed preferences are more consistently present. ACP also offers families direction on future care decision making which can alleviate burden. However, knowledge gaps exist regarding when to engage persons with dementia and care partners in ACP discussions and what areas of future care to focus on. This presentation will provide an overview of a series of studies that together examined the types of ACP conversations considered most pertinent for persons with dementia and their families, the challenges and opportunities of using ACP workbooks and other materials to support ACP processes and the current state of comfort and capacity to address ACP with persons with dementia.

Caregiver Wellness Workshop

This course will included guided discussions with many interactive exercises to provide caregivers with tools, techniques and strategies to effectively manage stress, engage in self-care, and prevent compassion fatigue and caregiver burnout. Participants will leave with helpful knowledge, resources, new skills, and a self-care toolbox to pull from as needed. We will explore the following… - Role of the caregiver - Self-identity - Compassion Fatigue, caregiver burnout and different types of grief - Effectively managing stress, demands, problem solving and communicating - Effectively engaging in self-care Self-assessment tools and worksheets - Self-Care Contract (setting realistic SMART goals and make a self-care promise)
